TUDCA is a water-soluble bile salt, which, in contrast to ordinary bile salts, attacks both water-soluble and fat-soluble ends and causes a cleansing effect. This is fine for the biological purpose of bile salts (they emulsify fats in the intestines to aid absorption), but when bile acids collect in the liver, a clinical stage called cholestasis that occurs when the liver is unhealthy, these bile salts damage cells by destroying membranes and they signal death to the cells. TUDCA and other water-soluble bile salts such as UDCA combat this toxicity and thus indirectly protect cells from death.

Additionally, it appears that TUDCA also reduces stress for each cell's ergastoplasm, organelles within cells that serve as a pathway from the nucleus to the cytoplasm and heal and aid in protein production. By reducing ergastoplasmic stress, TUDCA is associated with a wide range of beneficial metabolic effects such as reducing insulin resistance and diabetes and is a neurological protector. However, the use of TUDCY is preliminary, but it is relatively reliable for liver damage as it is used in the treatment of cholestasis (in hospitals).
